Facilitate continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders in adult beneficiaries. Provide patient education as an integral part of the services offered (e.g. medication education, alcohol effects on mood, etc.). Communicate with providers regarding patient status and documents appropriately in the electronic medical record. Coordinate patient referrals to support services provided through the internal and external community.
