Major Duties include, but are not limited to: Provides direct Veteran care and care coordination, as it pertains, to homeless Veterans and their care within the Health Care for Homeless Veterans Outreach Clinic. Manages assigned panel, utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. The HPACT RN provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Formulates a plan of care based on nursing assessments.
