The LPN is a key member of CLC they will contribute to the plan of care by collaborating with interdisciplinary team members, the patient and patient's family. Duties include but are not limited to: Demonstrates ability to accurately report and record significant patient information and uses good judgment in selecting the most appropriate application of procedures and guidelines, varying the order and sequence of assigned procedures based on the patient's condition.
