Duties include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Visits patients in their homes in the community. Drives a vehicle to patient visits in the community and follows relevant policies and procedures regarding operating a government vehicle. Assesses, identifies, and diagnoses patients' general medical problems and defines the medical management. Determines the need for specialty consultation.
