Duties from the announcement
This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until December 15, 2026. The initial cut-off date for referral of eligible applications will be October 30, 2026. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. **A recruitment and relocation incentive may be offered for a highly qualified applicant** Total Rewards of a Allied Health Professional Primary Care Health Integration (PCMHI) is a component of the interdisciplinary Patient Aligned Care Team (PACT).
