Make a meaningful difference in the lives of Veterans by helping them receive the care and support they need to remain in their homes and communities. As a Community Care Home and Community Based Services RN Case Manager at the Sioux Falls VA Health Care System, you will play an important role in promoting independence, preserving quality of life, and connecting Veterans with essential services. This position offers the opportunity to apply your clinical judgment, build collaborative relationships with interdisciplinary teams and community providers, and help Veterans navigate complex care needs.
